Ãå±±½ûµØ Laws and Rules Intertwining Schools and Vaccinations
These pages provide the text of the statutes which require children to be vaccinated. It also includes the regulations made by the Department of Public Health pursuant to the Ãå±±½ûµØ General Assembly delegating the authority to make rules. In short, the laws require all students to present proof of immunization to their schools.
Medical Excuse Policy
Health Services does not provide medical excuses for missed classes, exams, assignments, or work on/off campus. The decision for our policy is based on our limited resources, which are better dedicated to providing health care; the inability for us to make valid determinations about illnesses or injuries students may have managed through self care at home; and our commitment to student privacy.
Student Insurance
The UIS Student Insurance Plan is an affordable, high-quality insurance program, designed to meet the needs of our students, with optional coverage available for dependents. The insurance carrier for 2025-2026 is (UHCSR). The UHCSR website can be used to find information about benefits, providers, prescriptions, and more!

Immunization Requirements
Upload Immunization Records
Health Services will only be accepting immunization records that are uploaded to the . Log in using your NetID and school password then verify your birthdate. Select the Forms and Requirements tab on the left side of your home screen and follow the directions to upload your immunizations. If you need to submit a religious exemption for immunizations, that is also located on the patient portal under the Optional Upload Form tab on the left side.
